Summary:

The Patient Care Coordinator will provide support for delivering quality health care services to a diverse patient population. The position will serve as the portal to the specialty practice and be responsible for appropriate patient appointment making, record request and review, and maintaining relevant family history. 

They will coordinate the visit (appropriate provider, location, scheduling, referral in place) and answer patient questions, set expectations, and address concerns.

Using culturally and linguistically appropriate methods, the Patient Care Coordinator will work as a member of the health care team and interface and engage with all stakeholders, including providers, other staff, payors, patients, and patient families, as appropriate. The Patient Care Coordinator provides the link between patients and providers to maximize the patient experience, increase effective communication between providers, referring providers, patients and families, and increase the timeliness and efficiency of service delivery.

The Patient Care Coordinator will understand and interface with clinical, research, and quality databases.

Required Experience:

1) Assist patients and families in accessing and navigating medical care while understanding testing and treatment and partnering with the health care team, using culturally and linguistically appropriate methods. 
Ensure implementation of well-coordinated care plans for a continuum of care through collaboration with clinical teams.

2) Explain processes and responsibilities to families concerning their clinical care.  Assist patients and families with completing medical, financial, or other forms. Ensure timely access by triaging referrals with accurate data collection.

3) Documentation Responsibilities: Keeps process logs of patients reviewed, the dispensation of initial referral, provider scheduled, and nature of follow-up needed to serve the best patient. Maintain and update documentation of contact with the patient. Collect, enter, and compile data for program integration, reporting, and program evaluation—document services in medical records.

4) Maintain current knowledge of the specialty practice area.

5) Able to work independently while recognizing the importance of communicating with the clinical team and supporting team goals. 

6) Maintain and submit complete and accurate records, datasheets, confidential forms, and consent forms.

7) Ensure all mechanisms are in place to guarantee and optimize available reimbursement, including referrals, prior authorizations, and "compassionate care programs" before service. Act as a liaison with insurance payors.

8) Participate in the development of culturally and linguistically appropriate educational materials.

9) Participate as an active member of the clinical team to evaluate, measure, and implement care plans, protocols and to define roles to meet the goals of the specialty practice. 

10) Coordinate patient care using a patient-centered approach.

Minimally Required Experience

2-5 years in a medical setting. Ability to work in a multi-cultural, diverse, decentralized environment as a health care team member. 
Comfortable working with multi-cultural, low-income families with complex medical issues.
