Summary:
The Addiction Consult coordinator works as a collaborative team member for the Addiction Consultation Team of the Psychiatry Consultation Service. Collaborates with floor nursing staff, medical social workers, behavioral resource nurses, and referring providers to assure appropriate referrals to the service and timely response. Coordinates follow-up care for patients with SUD. Assures smooth transition to community-based services. Manages tracking and reporting as required. The coordinator demonstrates knowledge and experience with and treatment of SUDs. Must have excellent written and verbal communication skills.
The coordinator represents and supports the mission and philosophy of the Department of Behavioral Health, as well as the Addictions Consultation Team of the Psychiatry Consultation Service.
Fields triage calls to the service and serves as a liaison between medical and surgical providers and members of the Addictions Consultation Team. Provides input to patient care assessment and plan of care.

RESPONSIBILITIES:
1) Patient/Provider Engagement and Education: establishes and maintains a strong knowledge base about SUD and MAT, as well as community resources. Facilitates communication with psychiatry as needed and appropriate. Completes other administrative functions as needed.
2) Collaborates with hospital inpatient providers and staff on the care of referred inpatients with co-morbid SUDs. Responds to calls from physicians and other service providers promptly. Helps to clarify the specific services needed.
3) Acts as a primary resource to Case Management and Social Work colleagues department regarding patients with co-morbid SUDs who have been referred to the Addiction Consultation Team. Coordinates referrals to TAT services and disposition to outpatient or residential TAT settings.
4) Works closely with key hospital personnel regarding patients for whom disposition is problematic due to medical and SUD to bring about successful resolution and disposition for these patients through collaborative efforts.
5) Updates the team regarding the status of SUD patients on med/surg floors who may be awaiting outpatient or residential TAT disposition.
6) Maintains statistics for the ACT as needed.

Required Experience
1) Minimum 2 years experience in multidisciplinary behavioral health or medical social work/care management setting
2) 2 years working with individuals with Substance use disorders
3) 1 year of experience in a hospital or another medical setting
