Baystate Health, a nationally recognized leader in healthcare quality and safety, and home to Baystate Medical Practices, the largest multispecialty group in the region is looking for a full-time Medical Assistant to join our experienced ACO Care Management Team team in Springfield, MA.

RESPONSIBILITIES:

Baystate Health, ACO Care Management Medical Assistants are responsible for helping patients and their families navigate and access health and community services while adopting healthy behaviors. The ACO MA worked as a member of the care team through an integrated approach to care management and community outreaching, providing patient care and education. Responsibilities include working within the Care Management team setting, as well as providing follow-up outreach to ACO patients transitioning from a hospital setting back to their community. By managing patient lists, accurately and efficiently entering data, and following guidelines of follow-up care based on specific criteria set and quality measures.  Other responsibilities include, but are not limited to:

  • Provide outreach calls, per protocol, to patients that have been discharged from the hospital or the emergency room.  Review discharge instructions, ensure follow up appointments are made, screen for care management.
  • Population Management:  Utilize registries as well as standing orders to proactively identify and outreach to patients that are in need of follow up care/preventive screenings/chronic disease testing.  Assist patients in scheduling appointments and tracking results as needed.
  • Prescreens patients for baseline and/or complex care management utilizing a Health Risk Assessment.  Refer for enrollment into care management as appropriate.
  • Develop and maintain excellent working relationships with patients, ACO CM teams, physicians and their practice staff, as well as other community partners as appropriate.
  • Communicates and coordinates with all entities involved in the care of a patient to promote and maximize care coordination and patient satisfaction for a specific population of patients.
  • Follows up with patients that have been in the Emergency Department/Inpatient to facilitate PCP follow up and identify unmet needs, under the direction of the ACO care team.  Maintains clear, concise, objective, and accurate real-time documentation of patient encounters in electronic medical records.
  • Identifies, screens, and provides outreach to a specific subset of ACO patients to ensure needs for preventive care and key quality measures are met.  Works in collaboration with ACO CM teams, providers, and practices.
  • Communicates with community care partners to resolve any discrepancies regarding care plans and collaborates with community care partners on behalf of providers to coordinate patient services.

*This position is mostly administrative in nature. All work is done on phone or computer. May require travel to Community Health Centers in Springfield, MA.

SCHEDULE:

  • Full-time, 40 hours a week, Monday through Friday
  • 8:00am-4:30pm or 8:30am-5:00pm
  • NO weekends, NO holidays

QUALIFICATIONS: 

  • High School diploma or GED
  • Drivers license, preferred
  • Community Health Worker Certification, preferred
  • Registered/Certified medical assistant, preferred
  • Graduate of an accredited MA program or equivalent training with minimum 2 years of experience in medical clinical setting, required
