Our skilled and compassionate nurses and medical support staff offer an ideal combination of “high tech” and “high touch,” complementing an outstanding team of doctors. The newly-renovated Fowler Unit provides behavioral health services such as suicide screening and assessments.  External Hires:  SIGN ON BONUS of up to $13,500 for RN’s with 1+ year’s of experience and up to $8,000 for licensed New Nurse Residents.

Under the general supervision of the Director of Case Management acts as a patients advocate/case manager to hospital clients. An autonomous role that coordinates, negotiates, procures service and resource for, and manages the care of complex patients to facilitate achievement of quality and cost- efficient patient outcomes. Looks for opportunity to reduce cost while ensuring the highest quality and care is maintained. Applies review criteria to determine medical necessity for admission and continued stay. Provide clinically based case management, discharge planning, and care coordination to facilitate the delivery of cost-effective quality healthcare and assists in the identification of appropriate utilization of resources across the continuum of care. Works collaboratively with interdisciplinary staff internal to the organization. Participates in quality improvement and evaluation processes related to the management of patient care.

  • MA RN License Required
  • Diploma in Nursing required
  • Associates Degree Nursing preferred
  • Bachelor's Degree Nursing preferred
  • Basic Life Support – American Heart Association required
  • Prior Utilization Review, Discharge Planning or acute nursing care experience
  • Baystate Health required competencies per department
