QUALIFICATIONS:

Bachelor’s degree in Physical Therapy (Master’s preferred)

Massachusetts Physical Therapist Licensure

American Heart Association BLS CPR
POSITION HIGHLIGHTS:
-
BAYSTATE PHYSICAL THERAPISTS PERFORM AN IMPORTANT ROLE assisting patients in reaching maximum occupational performance while adjusting to disabilities or recovering from injuries. The OT completes timely, thorough assessment reports with problem-related treatment plan. Completes and updates interventions/reports in timely manner for MDT Treatment Plans. Enters concise progress notes in timely manner related to specific treatment goals. Completes discharge summary on time with specific recommendations.

Gathers all necessary information to assess functional status (ADL's, IADL's, leisure, daily structure, social skills, etc.). Successfully administers standardized testing. Comprehensively identifies strengths/assets. Clearly formulates clinical observations, problem-based treatment plan in accordance with assessment findings; clearly communicates results to MDT. Effectively collaborates with patient, other practitioners to establish overall treatment goals.

Establishes therapeutic relationship with patients (rapport, boundaries, etc.). Applies treatments in accordance with treatment plan, including precautions, contraindications. Appropriately manages pt. behaviors, related symptoms. Independently performs procedures and manages complex cases. Provides therapeutic individual and group treatment utilizing established OT, psychosocial modalities. Reassesses/reviews patient treatment and progress and consults/collaborates with MDT in timely manner in order to facilitate treatment plan updates. Collaborates with MDT to develop discharge plan and related follow-ups. Thoroughly orients students, new hires in accordance with program standards per supervisor assignment. Supervises COTA, students in accordance with program standards.
