Partial Remote Opportunity for a Telephonic Pediatric Triage RN
The Contact Center Triage Nurse– triages Pediatric and Primary Care patients telephonically to facilitate patients’ access to the appropriate level of care by assessing needs, offering a clinical option, referral to health care providers, services, and community resources. Promotes wellness and informed decisions by providing health information, home care advice, and prevention concepts.

The nurse ensures timely access to appropriate care and seamless coordination of services throughout the continuum. Works collaboratively with physicians and other health team members, and is available to patients and families for care coordination, will provide medical information and education to patient and families so they understand and comprehend the treatment and services rendered. The nurse will coordinate necessary appointments based on triage outcome as well as identify potential barriers to timely care and seek out appropriate resources for assistance.

  • Answers clinically related inbound calls. Maintains current knowledge base including triage skills, appropriate teaching principles, and policies and procedures.
  • Provides telephone triage of patient health status using general and emergency medicine (or risk stratification protocols) protocols. Aligns the right care for the patient, within the right time frame, and with the right provider.
  • Initiates all forms of communication for each encounter using accurate medical history and documentation, including patient education as well as reviewing test/procedure preps.
  • Partners and collaborates with patients, physicians, community services and all personnel as an integral member of the patient care team.
  • Resource to assigned scheduling team responding to clinical questions as relates to prescriptions, appointments, medical management, practice protocols and appropriate population management.

Required Work Experience: 

  • Massachusetts license as an RN and BLS CPR certified.
  • Minimum-3 years of nursing experience required
  • 2 years experience triaging Pediatric or Primary Care required
  • Ability to work in a fast-paced environment & handle multiple tasks/priorities
  • Excellent communication, interpersonal and problem solving skills to advocate for optimal patient outcomes.
  • Proficient in the use of multiple computer applications including EMR, spreadsheet programs and electronic medical records.
  • Must have competency in delivering exceptional nursing care and have proven success in partnering with both physicians and staff

Full -Time Monday – Friday-

Partial Remote opportunity after successful onsite orientation. Must be available to work on site as needed for ongoing training or to meet business needs
