Summary

The role of the team assistant is to support all clerical functions of the Pediatric Palliative Care Team and assure efficient delivery of care using effective communication and strong interpersonal skills.

Requirements

1) When necessary, answers phones and directs incoming calls, messages, and emails in  a timely manner using appropriate etiquette

2) Provides designated operational reports for managers and other stakeholders as assigned.  Monitors various forms and attaches to medical records in a timely manner. Prepares admission packets and assists in ordering forms as needed  

3) Monitors Forcura and processes incoming faxed documents

4) Maintains Pediatric Palliative patient databases

5) Assists Pediatric Palliative Care Manager and team members with clerical tasks as requested

6) Completes workflow tasks in EMR as designated with cross-training to other functions (intake, scheduling, other)

7) Assists volunteer coordinator as requested  

8) Attends weekly IDG meetings and enters notes in EMR

9) Assists with patient/family phone calls regarding scheduling, event reminders, and other tasks

10) Assists with patient mailings

11) Assists with the intake process

12) Responsible for mailing care plans to families

13) Maintains referral database & waitlist

14) Assists with family engagement / special events

15) Assists with ordering supplies and maintaining the supply closet

16) Responsible for preparing meeting agendas and minutes

17) Other duties as assigned

Required Experience

1) Minimum of 2 years experience, preferably in a healthcare environment

Preferred Experience

1) Spanish language skills

1) Strength in medical terminology

2) Computer/data entry experience

3) Excellent organizational and communication skills

4) Ability to handle multiple tasks

5)Excellent interpersonal skills and telephone etiquette

6) Ability to maintain confidentiality required
