DEPARTMENT DESCRIPTION:
Baystate Home Health interdisciplinary care teams consist of experienced nurses, therapists, social workers, hospice aides, spiritual and bereavement counselors, and volunteers. Care teams work together with both the patient and family, to ensure a safe and swift recovery from illness, accident, or surgery in the comfort of home. All care is expressly tailored to meet each patients’ needs.

POSITION RESPONSIBILITIES:
Home Health Registered Nurses provide nursing services in patient homes and assume responsibility for the care and well being of assigned patients. They play an integral part of the care team and contribute to the efficacy of care delivery and an overall positive patient experience.
In addition, the Registered Nurse will;

Act as a leader on the interdisciplinary care team, using current clinical knowledge to educate patient, family and staff on diagnosis, treatment program and resources available

Assess and revise goals and treatment plans as appropriate for high quality service to patients, in collaboration with care managers. The goals established will be realistic and inclusive of patient, family members and care team suggestions

Collaborate with managers to provide appropriate observations and experiences for students and orientees
-
Demonstrate knowledge of developmental stages for the applicable age groups (neonate infant, toddler, adolescent, adult, elder adult) and apply developmental theories and concepts when implementing care

Demonstrate current knowledge of pharmacology and medication administration.
QUALIFICATIONS:
-
Registered Nurse Certification with an Associate's Degree

Valid Driver's License, reliable vehicle and clean driving history
-
RNs in this position are required to demonstrate competency in IV and Phlebotomy within one year of their date of hire
-
One year of medical-surgical nursing with experience in community health care preferred
