Baystate Health, western Massachusetts’s premier healthcare provider, and home to Baystate Medical Practices, the largest multispecialty group in the region is looking for a Community Health Worker to join our experienced ACO Care Team at Greenfield Family Medicine in Greenfield, MA.
RESPONSIBILITIES. Baystate Health Community Health Workers perform an important role providing care to our patients. Helping patients and their families navigate and access health and community services whiling adopting healthy behaviors. Works as a member of the ACO care team through an integrated approach to care management and community outreach. Promotes, maintains, and improves the health of patients and their family by providing social support and informal counseling. Partners closely with different providers, patients and outside organizations and is responsible facilitating communication between patient and care teams; and providing culturally appropriate health education and self-management support. Additionally, Community Health Workers will collaborate within a team-based care delivery model to improve the health and lives of our community members. Community Health Workers participate in the development and implementation of innovative costs-effective delivery system changes that proactively meet the complex needs of our patients including efforts to manage their behavioral and social needs in addition to their medical care.
SCHEDULE. Full time: Monday thru Friday. No weekends, no Holidays.

Requirements:
o High School diploma or GED
o Community Health Worker Certification, preferred
o Bilingual, preferred
o Valid drivers license
o Great interpersonal skills, compassionate care giver and able to work in a fast paced environment a MUST.
