POSITION HIGHLIGHTS:

The Care Management Supervisor (sometimes referred to as a Case Management Supervisor) is responsible for educating, monitoring, assessing, and directing the daily work of the RN case managers and utilization management nurses.

In addition, the Care Manager Supervisor will:

  • Lead the hospital care management team, who is responsible for the coordination of clinical care, post-acute discharge disposition, and financial outcomes of a designated population of patients.

  • Facilitate patient care plans

  • Coordinate input from all health care professionals, and conduct assessment of patient and family needs to facilitate an optimal discharge plan assuring continuity of care.

  • Identify and contribute to modifications in nurse and physician practice patterns, utilizing case management tools to achieve quality of care, patient satisfaction, and appropriate use of resources, in alignment with medical management initiatives and product line goals.

  • Evaluate appropriateness of admissions and continued stay
