Responsible for overall management of a bereavement program. Responsibilities include the planning, implementation, maintenance and provision of services necessary to patients, identified caregivers, hospice staff, and the community at large. Hospice bereavement services extend for the time period of the patient's admission to hospice services until at least thirteen months after the death of the patient.
Provides grief and bereavement support to patients and families consistent with needs of the patient. Serves as a member of the Interdisciplinary Team to develop the plan of care. Acts as liaison between community and hospice program. Adheres to system and department compliance policies, and any and all applicable laws and regulations. Performs other duties as assigned.
1) Provides oversight of bereavement program. Write and oversee printing and mailing of bereavement newsletter. Orient bereavement volunteers – both for office support and bereavement companion volunteers. Coordinate all aspects of bereavement support groups. Provides oversight and preparation of hospice memorial services
2) Develops and provides grief/bereavement education and support for any hospice patient/family members who requests as well as requests from the community and other individuals. Acts as a liaison and resource for local community/Regional Bereavement Coordinators and Regional Volunteer Coordinators.
3) Provides patient and/or family grief counseling, as requested as well as ongoing supportive communication with active bereavement volunteers.
4) Develop and present bereavement related programming in request to community and hospice program needs.
5) Demonstrates and maintains current knowledge and skill in providing appropriate intervention for patients in all age groups: Geriatrics, Adults, Adolescents, Pediatrics.
6) Maintains professional practice during stressful or crisis situations.
7) Participates as active member of Interdisciplinary Team meeting to establish, review and revise the plan of care, including exchange of information, review of problems, and sharing of professional support. Make decisions in consultation with Interdisciplinary Team. Collaborates with Interdisciplinary Team to develop and implement comprehensive bereavement plan of care. Relays information regarding patient to appropriate team members and displays and promotes teamwork. Collaborates with team to provide grief and bereavement support to patients and families. Adheres to system and department compliance policies, and any applicable laws and regulations.
8) Communicates clearly and effectively, fostering a positive, proactive environment. Inspires cooperation and achievement by treating others in a respectful professional manner. Adheres to high standards of ethics and quality. Maintains files and documents necessary and pertinent information regarding patient/family in the patient's record in accordance with regulatory and licensure requirements. Responsible for maintenance of electronic medical records, assuring accuracy, completeness, and compliance with regulations, licensure standards, legal and ethical standards.
9) Attend orientation, in-service, staff and any other departmental, agency or system-wide programs/meetings as requested.
10) Participate in group memorial services for staff, the bereaved and the community.
11) Supervise bereavement volunteers and trains volunteers to assist with bereavement functions.
Required Work Experience:
1) Documented evidence of training and experience in grief therapy and bereavement counseling helpful
2) Recent (within the past 5 years) experience in acute, long-term care or home care/hospice, with at least one in hospice preferred
3) Experience and professional training in grief and bereavement care preferred
4) Understanding of physical, psychosocial and spiritual needs of terminally ill patients and their caregivers
